A referral is not completed care.

  • 66.5%

of Medicaid mothers had a qualifying postpartum visit (state median, ages 21+).

Source: CMS Adult Core Set, 2025

  • 53.8%

of patients referred from maternal-fetal medicine attended their first primary-care visit.

Source: Malhotra et al., Women's Health Reports, 2025

When we did this by hand for one mother, we contacted about 40 providers and spent 48 to 50 hours to book a single consultation. FIELD’s agents now do that work.

57% vs 30%

Follow-through works. In a randomized trial of 405 postpartum patients, those with a navigator attended primary care at nearly twice the rate by 11 to 13 months.

Agents do the follow-through. People handle the exceptions.

The repetitive work between a referral and a visit is mostly searching, calling and checking back. FIELD does that work, every time, until the visit happens.

  1. Step 1: Find & verify

Reads the referral, asks what works for her, and finds providers who are in network and seeing new Medicaid patients.

    Agent

  2. Step 2: Call & book

Calls the practice, gets a real appointment slot and confirms it with the mother.

    Agent

  3. Step 3: Recover when options fail

No answer, no openings, a cancellation: FIELD moves to the next option and calls again.

    Agent, then care team

When it’s stuck or needs judgment, a person takes it.

No provider will take her plan. She can’t be reached after several tries. Something she says needs a clinician. A FIELD care coordinator picks up the case with the full history, so nobody starts over.

  4. Step 4: Confirm the visit

Sends reminders, checks in the day before, and reports the visit back to the plan or practice.

Extend your care team’s capacity.

FIELD handles the repetitive execution. Your nurses, navigators and doulas get the cases that need a human.

How a pilot works

  1. Step 1: See FIELD

A walkthrough and a live call on a workflow you already run.

  2. Step 2: Choose the workflow

One population and one gap, such as postpartum visits for a set of members.

  3. Step 3: Measure the value

Agree on the measures up front: visits booked, visits attended, cases escalated.
